This form should be submitted by the onsite main contact for each team NO LATER THAN Friday, March 13th.
The contact mobile phone number for your team should be a coach/mentor that will be at the event – We need this information to contact teams in case of emergencies, and it does no good for us to call your school.
Please submit all fields of this form for all events your team is participating at no later than two weeks before each specific event.
